What is Bolton Health and Care Partnership

We are a partnership of organisations working together to radically change the way we deliver health and care services in our town.

In 2018, Bolton Council, Bolton NHS Foundation Trust (FT) and NHS Bolton Clinical Commissioning Group (CCG) announced plans to form an integrated partnership with Greater Manchester Mental Health Foundation Trust (GMMH) and the voluntary and community sector which would deliver the health and care system for the town.

Who’s involved

Bolton Health and Care Partnership is an integrated team made up of professionals and clinicians across the following five organisations:

Bolton Council
Bolton NHS Foundation Trust
NHS Bolton Clinical Commissioning Group
Greater Manchester Mental Health NHS Foundation Trust
Bolton CVS

These organisations with other health and social care partners, as well as the voluntary and community services are developing new approaches of working together to improve patient outcomes.

Why do we want to do this?

Working together across all organisations is a great way to make the best use of the services we have and helps to avoid duplication.

How will we do this?

There are two main elements of the Bolton Health and Care Partnership: how we pay for health and care services across Bolton and how we deliver them.

Our Integrated Care Partnership

The Integrated Care Partnership (ICP) is a partnership made up of the different providers of health and care in Bolton.

How we buy services

From April 2019, the budgets of Bolton Council and Bolton CCG to buy health and care services for adults and older people were pooled to create a total fund of around £160 million.

The Neighbourhood Model

The neighbourhood model of care in Bolton is based on one locality, three districts, and nine neighbourhoods – each with a population of approximately 30,000-50,000.

How the neighbourhood model will work

Each of the nine neighbourhoods will have a neighbourhood team working in an integrated way to improve health and wellbeing in the community, working to involve and empower local residents.
