What is Bolton Health and Care Partnership

We are a partnership of organisations working together to radically change the way we deliver health and care services in our town.

In 2018, Bolton Council, Bolton NHS Foundation Trust (FT) and NHS Bolton Clinical Commissioning Group (CCG) announced plans to form an integrated partnership with Greater Manchester Mental Health Foundation Trust (GMMH) and the voluntary and community sector which would deliver the health and care system for the town.

Bolton Health and Care Partnership is an integrated team made up of professionals and clinicians across the following five organisations:

These organisations with other health and social care partners, as well as the voluntary and community services are developing new approaches of working together to improve patient outcomes.

How we buy services

From April 2019, the budgets of Bolton Council and Bolton CCG to buy health and care services for adults and older people were pooled to create a total fund of around £160 million.

The neighbourhood model of care in Bolton is based on one locality, three districts, and nine neighbourhoods – each with a population of approximately 30,000-50,000.
